Capricorn Healthcare (“Capricorn”) announced that it closed a transaction with Healthy Living at Home (“HLH”), a leading provider of home health, hospice, and private duty nursing across four states in the Western US. Through its investment, Capricorn will partner with HLH’s current owners and executive team, who will continue to lead the company. Financial terms of the transaction were not disclosed.

Based in Northern California, HLH delivers care to a broad spectrum of patients in their home, including skilled nursing, rehabilitation, private duty care, and hospice. The Company’s 30+ local agencies provide services to more than 3,500 patients daily. HLH successfully manages thousands of patients’ transition from acute care settings into the home each year, working with health systems, skilled nursing facilities, and physicians in the community to drive quality outcomes.

Capricorn Healthcare (“Capricorn”) announced that it closed a transaction with Altruista Health (“Altruista”), a care management and population health software platform serving payer and provider organizations. Through its investment, Capricorn will partner with Altruista’s co-founders and executive team, who will continue to lead the company. Financial terms of the transaction were not disclosed.

Based in Reston, VA, Altruista Health delivers web-based medical management and care coordination applications that help healthcare organizations address the complex care and support needs of Medicaid, Medicare, and other high-risk populations. The company’s GuidingCare™ software platform includes several integrated modules that consolidate key population health management functions, including predictive risk modeling, gaps-in-care analytics, care management, disease management, utilization management, physician/provider coordination, and member/caregiver engagement among others.  Altruista has rapidly become an industry leader.
